Health connect empowers health plan members to take more responsibility for managing their health care. Kaiser subscribers can access Health Connection through a web portal at Kp.org. Here they can view most of their personal health data online, including lab results, medication history and treatment summaries. Patients can enter their own reading from blood pressure and blood sugar monitors. They can also securely email their healthcare providers, reducing the time patients spend waiting to speak to a doctor and the number of office calls.
