The statistics of accidents on construction sites prove that the Malaysian construction industry is one of the critical sectors that need to take construction site safety practices seriously. The accident is not a natural occurrence. It happens due to unsafe acts, unsafe conditions, or both. In general, accidents are a combination of contributing factors, unsafe acts and unsafe conditions. The root causes of construction accidents are the main thing that needs to be highlighted to improve the overall safety performance. This knowledge could be used to create more favorable working conditions and environments on construction sites. Therefore, a study was conducted to identify the causes of accidents on construction sites. This study was initiated by examining literature from magazines, books, newspapers and web pages. Reported accident cases maintained by the Department of Occupational Safety and Health Malaysia (DOSH) were then examined to investigate the causes of the accidents. Surveys were conducted using questionnaire forms to obtain data from the respondents who are mainly contractors and the rest are development and consultancy companies across the countries who are experts in construction safety. The findings of this study reveal that accidents are the result of numerous factors, causes and sub-causes. Some of the critical factors are unsafe methods, human element, unsafe equipment, workplace conditions, management and unique nature of the industry. The causes of accidents in Malaysia were found to be similar to those mentioned in the literature review. However, some of the causes have a low frequency of occurrence.
