The statistics of accidents on construction sites prove that the Malaysian construction industry is one of the critical sectors that need to take construction site safety practices seriously. The accident is not a natural occurrence. It happens due to unsafe acts, unsafe conditions, or both. In general, accidents are a combination of contributing factors, unsafe acts and unsafe conditions. The root causes of construction accidents are the main thing that needs to be highlighted to improve the overall safety performance. This knowledge could be used to create more favorable working conditions and environments on construction sites. Therefore, a study was conducted to identify the causes of accidents on construction sites. This study was initiated by examining literature from magazines, books, newspapers and web pages. Reported accident cases maintained by the Department of Occupational Safety and Health Malaysia (DOSH) were then examined to investigate the causes of the accidents. Surveys were conducted using questionnaire forms to obtain data from the respondents who are mainly contractors and the rest are development and consultancy companies across the countries who are experts in construction safety. The findings of this study reveal that accidents are the result of numerous factors, causes and sub-causes. Some of the critical factors are unsafe methods, human element, unsafe equipment, workplace conditions, management and unique nature of the industry. The causes of accidents in Malaysia were found to be similar to those mentioned in the literature review. However, some of the causes have a low frequency of occurrence. The main cause of construction accidents found is workers' negligence, workers' failure to observe work procedures,......middle of paper......ntion, the accident rate will increase so it will affect seriously affects the company's image in the construction sector. On average, more than 40 accidents were recorded every year from 2005 to 2009, despite efforts to ensure environmental safety. All accidents have their causes, so it is important to identify the causes of the accident so that it can be prevented in the future and thus reduce injuries, illnesses and also costs for the company. This is the article I took from my sinchew.com, the title was the collapse of the second Penang bridge. The possible causes of the accident are due to these three factors, namely design, material used and quality of work not in accordance with specifications, as well as supervision and quality control on the project site. The safety manager should regularly monitor the construction site to prevent this type of accident from happening again in the future.
