This 5-year-old girl presented to the emergency room of our hospital complaining of blood-stained tears and pain. The attending emergency doctor called me urgently for examination and management of the case. During the time it took me to reach the hospital; the emergency medical officer had irrigated the patient's conjunctival sacs with normal saline and applied Polyfax eye ointment. The patient was brought here by her parents. According to the parents, the patient was seen by a general practitioner in his clinic for fever, who on examination found that her eyes were slightly congested and prescribed Metachlor eye ointment (chloramphenicol 5 mg/g + dexamethasone 1 mg/g) and a Novofenicol eye drop (chloramphenicol 0.5%). He was also prescribed Cefim DS syrup (Cefixime 200 mg/5 ml) and Brufen syrup for fever. Nearly 30 minutes after instilling the first drop and applying the ointment, her mother saw blood coming from her eyes. He had no ocular or family history. He had no known drug allergies. His family medical history was unremarkable. His medical history was significant for fever in the last 24 hours for which his parents had sought general medical advice. The patient, as well as her parents, were concerned about the condition and the patient was uncooperative during the examination. His visual acuity was 6/7.5 (0.8 decimal) OU. His extraocular motilities were full with no evidence of heterotropia or heterophoria. His pupils were equally round and reactive without any RAPD. His eyelids were slightly swollen externally. On slit lamp biomicroscopy there were mixed papillary and follicular reactions, OU units of inflamed crunculi, and multiple units of small subconjunctival hemorrhages.
